By Hand submission:

• Forms and prospectus are available at the “admission office”.
• Pay the application fee to the admission officer. (Admission fee Rs.2000)
• Fill the form and attach all the required documents.
• Submit form along with all required documents to the admission officer.

Online Submission:

• Forms are available on the “Website”.
• Download and fill the form.
• Pay the application fee Online to the following Bank Account:

• Email the ONLY ADMISSION FORM along with ONLINE FEE TRANSFER PROOF on fmcn@mashopital.org NO NEED to attach the EDUCATIONAL DOCUMENTS with admission form.
• Send all the educational documents on the following postal address:
